Applications can be picked up during office hours from 8:00 AM to 5:00 PM, Monday through Friday at 1901 Broadway Road, Pekin, Illinois. Applications are accepted on the second, third, and fourth Thursdays of every month from 8:00 A.M. to 5:00 P.M., except on holidays. The dates applications are accepted during 2008 are:

April 10, 17, 24 May 8, 15, 22 June 12, 19, 26
July 10, 17, 24 August 14, 21, 28 September 11, 18, 25
October 9, 16, 23 November 13, 20, 26* December 11, 18, 25

*Falls on Wednesday instead of Thursday
